Brooks to Bays Nature Tours                                                      Waiver of Liability I ______________________________, am joining Brooks to Bays Nature tours for a paddling tour in sound physical and mental health, or having made owner/operator, Jeremy Brooks, aware of any and all health concerns I have prior to this tour.  I realize the potential risks involved in water-based recreation, as explained by owner/operator Jeremy Brooks.  Any questions regarding rules and safety concerns have been asked and answered to my satisfaction.  I understand that I play an important role in my safety on and off of the water, as well as the safety of all other paddlers on this tour.  Following rules means a fun and safe tour for all. I WILL DO THE FOLLOWING TO ENSURE MY PERSONAL SAFETY AND THE SAFETY OF OTHERS ON THIS TOUR… wear my PFD (personal floatation device) at all times while in a watercraft, and will not remove it until    out of the craft and on solid, stable ground stay as close to the group as possible and will stay with my “buddy” use appropriate hand signals taught to me for on-the-water safety GIVE NON-VERBAL, WRITTEN PERMISSION for First-Aid + CPR certified owner/operator, Jeremy Brooks, to apply necessary First-Aid and CPR if I am injured, unconscious, and unable to respond verbally follow all instructions and abide by all rules explained by owner/operator, Jeremy Brooks while in his supervision on and off of the water consume only non-alcoholic beverages while on the tour and will not paddle if I have consumed alcoholic beverages within 3 hours of the tour I WILL NOT HOLD OWNER/OPERATOR, JEREMY BROOKS, LIABLE FOR THE FOLLOWING… arriving late or missing my tour my decision to take off my PFD at any point while in a watercraft and any physical harm or death to myself or others resulting from a lack of a PFD physical harm or death to anybody incurred as a result of reckless, careless, or dangerous behavior while in, around, or using any watercraft or other tour-related equipment any damage or loss of personal equipment any illness or allergic reactions (food allergies must be made known prior to tour) any symptoms resulting from weather exposure – i.e. sunburn, cold, dehydration, sun poisoning, heat stroke anything that may happen to myself if I leave the group during the tour any physical harm to myself caused by interactions with any other tour member BY SIGNING THE SPACE BELOW, I OFFICIALLY STATE THAT I HAVE READ AND UNDERSTOOD THIS WAIVER.  I WILL TAKE ALL MEASURES TO ENSURE MY SAFETY AND THE SAFETY OF THOSE AROUND ME.  I WILL NOT HOLD OWNER/OPERATOR, JEREMY BROOKS, LEGALLY OR PERSONALLY RESPONSIBLE FOR ANY DAMAGE OR PHYSICAL HARM CAUSED BY MY FAILURE TO COMPLY WITH THE TERMS OUTLINED IN THIS WAIVER.  I ALSO UNDERSTAND THAT FAILURE TO COMPLY WITH THESE TERMS WILL RESULT IN IMMEDIATE DISMISSAL FROM THE NATURE TOUR WITH NO POSSIBILITY OF A REFUND OF ANY PAYMENT. _________________________________    __________________________________    ______________ Print Name                                                     Signature                                                         Date : mm/dd/yy
